RESURFACING HIP ARTHROPLASTY +*Hananouchi, T; *Sugano, N; *Nishii, T; *Sakai, T; **Miki, H;**Lee, SB; **Ohzono, K; *Yoshikawa, H +*Department of Orthopaedic Surgery, Osaka University Graduate School of Medicine [email protected] Introduction: Metal on metal resurfacing hip arthroplasty (RHA) has been getting popularity due to its concept of bone conservation, stability, and expected longevity. However, postoperative femoral neck fracture is the most serious complication in RHA and the pathomechanism of the fracture is still controversial[1-3]. Some reports suggested that the femoral neck fracture after RHA is derived from avascular necrosis of the femoral head due to the operative procedures[4,5]. Although it has been reported the blood flow in the femoral head was decreased by the femoral preparation or femoral notching, the vasculature of the femoral head remnant (FHR) after RHA has not been visualized yet. The purpose of this study was to investigate the effect of femoral procedures on the vasculature of FHR quantitatively with a micro CT after RHA using resected femoral heads and plastic replica femoral implants.

Notching of the femoral neck during resurfacing arthroplasty of the hip: a vascular study.

During hip resurfacing arthroplasty, excessive valgus positioning or surgical technique can result in notching of the femoral neck. Although mechanical weakening and subsequent fracture of the femoral neck are well described, the potential damage to the retinacular vessels leading to an ischaemic event is relatively unknown. Using laser Doppler flowmetry, we measured the blood flow in 14 osteoa...

Osteonecrosis in retrieved femoral heads after failed resurfacing arthroplasty of the hip.

We present the histological findings of bone retrieved from beneath the femoral components of failed metal-on-metal hip resurfacing arthroplasties. Of a total of 377 patients who underwent resurfacing arthroplasty, 13 required revision; for fracture of the femoral neck in eight, loosening of a component in three and for other reasons in two. None of these cases had shown histological evidence o...

Hip Resurfacing Arthroplasty in Treatment of Avascular Necrosis of the Femoral Head

BACKGROUND Hip resurfacing is a conservative type of total hip arthroplasty but its use is controversial, especially in patients with osteonecrosis. The aim of this study was analysis of the clinical and radiographic outcomes of hip resurfacing in patients with osteonecrosis. MATERIAL/METHODS Between 2007 and 2008, 30 hip resurfacing arthroplasties were performed due to osteoarthritis seconda...

A simple and reliable technique for placing the femoral neck guide pin in hip resurfacing arthroplasty.

Early failure of hip resurfacing has been attributed to component malposition and other factors. Accurate placement of the femoral guide pin is challenging with commercial guides, and computer navigation adds considerable cost to the procedure. Precise pin placement can be achieved by drilling a 2.8-mm pin across the hip joint in a retrograde fashion similar to placing a dynamic hip screw pin. ...

"Tripolar" hip arthroplasty for failed hip resurfacing: nineteen years follow-up.

The authors describe the case of a 37-year-old patient who sustained a subcapital femoral neck fracture six months after ICLH double-cup hip resurfacing. As the polyethylene acetabular resurfacing component was undamaged and well fixed, a standard femoral stem with a bipolar head was inserted. The outer diameter of the bipolar head was chosen to fit the resurfacing socket. The "tripolar" hip ar...
